Here's an overview of the process to get your medical marijuana card

IMPORTANT- Bring the following with you to your appointment:

 

Completed forms (if you printed them yourself)

Photo ID of patient

Photo ID of caregiver (if needed)

Photo ID of grower (if needed)

Clinic Fee of $175

Step 1-In order to obtain your Oregon state medical marijuana card is to have a qualifying condition.  According to the OMMP handbook, current qualifying conditions include:

 

 

Qualifying debilitating medical conditions

Under the OMMA, uses of medical marijuana for debilitating medical conditions are defined as:

  • Cancer;

  • Glaucoma;

  • Agitation due to Alzheimer’s disease;

  • HIV+/AIDS.

 

Also, any other medical condition or treatment for a medical condition that produces one or more of the following:

  • Cachexia (a weight-loss disease that can be caused by HIV or cancer),

  • Severe pain

  • Severe nausea

  • Seizures (including but not limited to seizures caused by epilepsy)

  • Persistent muscle spasm ( including but not limited to spasms caused by multiple sclerosis)

If you think you have a qualifying condition then continue to step two.

 

Step 2- Print and complete the Records Request form found here.

 

Step 3- Once we receive your records, our doctor(s) will review your records to determine if you have a qualifying condition.  We will then call you to set up your appointment date and time.

Step 5- Complete the necessary forms for the day of your appointment.  You can print these forms and bring them with you already completed OR you can complete them prior to your appointment at our office.  Please note, if you chose to complete these forms at our office prior to your appointment; please come 30 minutes early and tell us you need the forms when you check-in. 

 

 

Step 6- After your appointment you must mail and submit your application to the Oregon Health Authority for your official card.  You will receive a Compliance Packet with instructions at your appointment with specific instructions on how to be able to legally use and poses your medication immediately after your appointment. 

 

 

IMPORTANT- The following fees are payable to the State in addition to the $175 clinic fee.

 

Normal Application Fee:   $200.00

 

Reduced Fees: (You can pay a reduced fee if you receive any of the following benefits. You must send a copy of proof of benefits with your original application.)

OHP- $50.00                       SNAP-$60.00                     SSI- $20.00

 

Grower Fee: $ 50.00 (You must pay unless you are your own grower.)
